COACH AND PLAYER EJECTION FORM

If you find it necessary to eject a coach or player from an athletic contest, it is your responsibility to call your designated association representative immediately following the game, and follow that call by either faxing or emailing this form to the CIFSDS Office and to your association representative within 12 hours of the ejection. It does not matter if: (1) The coach is a head coach or an assistant; (2) The level is varsity, JV, or Frosh; or (3) The team is from the San Diego Section or not.
