When filling out the CIF Coach & Player Ejection Form refer
to Disqualification Report
Instructions. If you download and save the form be sure you
use a unique name as described in Disqualification Report Instructions.
Make sure you email or fax your report to Bob Petterson by noon
the next day, (Monday, for Friday and Saturday games).
Below are links to the ejection (sendoff/red
card) form on our web server. If you open a form and there is
no "back" or "home" link, just close the form
to return to this page. If you are filling it out from your pc
simply type your name on the signature line.

This opens as
an editable form in MS Word. After clicking on the link you
will see a log in box - just click cancel and the form will
open. After it opens you can either (1) print it, fill it
out by hand, then fax or scan it, or (2) fill it out on your
pc, save it to your HD, then email it as an attachment. It
needs to go to Bob Petterson, scsoasd@yahoo.com fax 858-695-3457.
