opens fill in the boxes and spaces to create your report.
Then click File, Save As, and save it to your documents folder
(or wherever you keep your files). Then send it as an attachment

Effective February 7, 2005 any coach, player, team attendant,
or spectator ejected from any contest for any reason, at any level,
is suspended indefinitely from participation, practice, or attending
any sports contest, until a meeting can take place between the
school administration, coach, player, parents/guardians, and CIF
staff member(s). Following that meeting, a determination
of penalty will be determined and there will be no appeal.
Therefore, please used your best professional judgement concerning
game control and the issuance of cards. When red cards are
appropriately issued, a complete report, on the proper form, is
to be sent to both CIF and SCSOASD within 12 hours.
